​Roots Perinatal Resource Guide Application   ​

Thank you for joining the growing number of providers in southwest Montana who are expanding their ability to work with families in the perinatal period. The Roots Resource Guide: Connection and Support from Pregnancy to Parenthood highlights those providers who have received specific training in Perinatal Mood and Anxiety Disorders (PMADs) as well as those who have experience working with families in this unique developmental stage. It will serve as a safety net for families seeking care and providers seeking referral options. This Guide will not serve as a recommendation or endorsement of specific providers, but will simply serve as an informational listing for those seeking support.

​Steps to be listed in 2023: 
​

  • Review eligibility requirements & fees, completely fill out the application, and submit payment below.

  • Your application will be reviewed by the Resource Advisory Committee and you will be contacted if they have any additional questions.
​
  • If your application is approved, you will be notified and your listing will be added to the guide and printed in January 2022. If your application is not accepted, you will notified and will be refunded your application fee.

The DEADLINE to submit your application to be listed in 2023 is OCTOBER 27!

​If you feel you have professional experience but do not meet all of the criteria, please contact the editors at 406-570-0738 to discuss where your experience may fit.

FAQ before getting started:
1. I am in practice with other providers. Should we fill out one application or separate applications? 
We have tried to streamline the application process so groups of providers working together can all apply directly on the same application, however, that does require that you have all of your colleagues' information handy when submitting, i.e., # of years in practice, CEUs/names of specific PMAD trainings/pertinent information about how PMADs have been addressed in their training. You can always apply separately if that's easier than wrangling information and send us an email to be invoiced as a group.

2. I do not meet the CEU requirements but I have been working with moms and families for a long time and feel I offer a valuable service and should be listed. What should I do? 
Apply! We value providers who have experience working with perinatal clients. There is a place in the application to expand on how you work with moms and families, and how PMADs have been addressed in your professional development. Feel free to share more with us about how you will use this guide to refer your clients, etc. Please contact us if you have further questions.

3. I am not currently accepting new patients/clients, should I still list in the Guide? 
We have received numerous questions about this because like you, we certainly never want to send a parent down a rabbit hole of seeking care to find out that no one is accepting patients, BUT we also know that the nature of availability is dynamic. If you/your practice has made a commitment to addressing perinatal mental health, our community needs to know. 
4. I am not a direct service provider but I offer free support groups or other opportunities for mothers and families to come together. Should I be in this resource guide? 
Absolutely! Not only is this guide meant to be a place to find healthcare professionals working in the perinatal period, it is meant to be a place where all the resources in our community that support the mental well being of families can be highlighted.     Listing Information 
    Our goal is to make it EASY for parents reaching out for support. PLEASE consider this when filling out your application and provide us with the most up to date information and best way for someone to reach you. For example: do you accept text messages? 
    Max file size: 20MB
    ​In 75 words or less, please share ( in third person) how you care for mothers and families, including how you address perinatal mental health in your office. Please be specific. For example, do you have a protocol for screening and referral? Do you have a Behavioral Health Professional on staff? Do you or your staff have training to work with/understand the greater challenges faced by BIPOC and LGBTQIA+ families? Do you work with partners/dads? Do you have a solid understanding of how to use this guide to refer your families to the care they need?
